POWER OF ATTORNEY
Prepared October 23, 2026 · Ontario, Canada
RECITALS
WHEREAS the undersigned desires to set out in writing the matters described below;
WHEREAS this document has been prepared with reference to the laws of Ontario, Canada;
NOW, THEREFORE, the following provisions shall apply:
October 23, 2026
In plain language: Jordan Miller gives Alex Lee the legal authority to make decisions about property and financial matters on their behalf. This is a powerful document — the attorney can act as if they were you for the matters listed below.
ARTICLE 1 — GRANTOR
I, Jordan Miller, of 123 Example Street, Toronto, ON M4B 1B3 (the "Grantor"), being of sound mind, appoint the person below as my attorney.
ARTICLE 2 — ATTORNEY
Alex Lee, of 456 Second Ave, Ottawa, ON K1N 5B6 (the "Attorney").
Alternate attorney: if Alex Lee is unable or unwilling to act, I appoint Taylor Brooks as alternate attorney with the same powers.
ARTICLE 3 — SCOPE OF AUTHORITY (PROPERTY AND FINANCIAL MATTERS)
The Attorney is authorized to manage all property and financial affairs, including banking, investments, real estate transactions, paying bills and filing taxes. The Attorney must act honestly, in good faith, and solely in the best interests of the Grantor, must keep the Grantor's property separate from their own, and must keep accurate records of all transactions.
ARTICLE 4 — WHEN IT TAKES EFFECT
This power of attorney takes effect IMMEDIATELY upon signing and continues in effect if the grantor later becomes incapable (it is intended to be an enduring/continuing power of attorney).
ARTICLE 5 — LIMITS
This power of attorney does not authorize the Attorney to make or change the Grantor's will, vote on the Grantor's behalf in an election, or delegate this authority to another person. The Grantor may revoke this document at any time while mentally capable, by written notice to the Attorney.
ARTICLE 6 — GOVERNING LAW
This document is governed by the laws of Ontario and the federal laws of Canada applicable there.
ONTARIO NOTE: Ontario uses two separate documents under the Substitute Decisions Act, 1992 — a Continuing Power of Attorney for Property (finances) and a Power of Attorney for Personal Care (health and personal decisions). If you selected "both" above, consider preparing both documents. A Continuing Power of Attorney for Property survives incapacity only if it expressly says so.
